Assessment of patients with hip fracture treated with hemiarthroplasty and internal fixation method

Objective:

Assessment and comparison of 137 patients who were treated with internal fixation and hemiarthroplasty in our clinic between May 2002 and December 2006 because of proximal femur fracture in terms of functional outcomes, mortality, complication and reoperation ratios.

Material and Methods:

A total of 137 patients were included in the study, 92 patients were detected to be alive. Group 1 number of the patients who underwent internal fixation because of hip fracture was 44 (24 females, 20 males, mean age 66,6±17,2 years; range 27-95). Mean follow-up time was 23±10,8 months (6-51). Group 2 number of the patients treated with hemiarthroplasty because of hip fracture was 91 (60 females, 31 males, mean age 73,8±11,2 years; range 49-96). Mean follow-up time was 22,7±15,4 months (6-54).

Results:

According to Harris hip score, excellent and very good outcomes were obtained in 18 (40,3%) patients who underwent internal fixation group, good and moderate outcomes were obtained in 19 (43,9%) patients and poor outcomes were obtained in 7 patients. Of the patients who underwent hemiarthroplasty group, outcomes were found as excellent and very good in 28 (45%) patients, good and moderate in 27 (43,5%) patients and poor in 7 (11,5%) patients.

Conclusion:

Although complication and reoperation ratios were found high in patients with hip fracture treated with internal fixation method, ratio of mortality was lower compared to hemiarthroplasty. Also functional outcomes of the patients treated with internal fixation and hemiarthroplasty are similar. Thus, preferring the treatment conserving hip joint would be more appropriate especially for the patients with femur intertrochanteric fracture.
