Meckel’s diverticulum can be symptomatic only when a complication develops. The most common complication is intestinal obstruction in adulthood. Strangulation of Meckel’s diverticulum in a hernia is a rare condition. Strangulation in a paracolic hernia is also seen infrequently. We report a case of a 80-year-old male patient with signs and symptoms of mechanical intestinal obstruction. Depending on clinical and radiologic data of the patient, emergency surgery was performed. In operation, we detected a Meckel’s diverticulum strangulated in paracolic hernia and necrosis of bowel wall adjacent to diverticulum. The necrotic bowel segment containing Meckel’s diverticulum was resected. Paracolic hernia was repaired. Patient died on the second postoperative day due to non-surgical problems.
